The basics

What is shoulder pain?

Shoulder pain is discomfort around the joint that often shows up when reaching overhead, behind the back, or lying on that side at night.

Most shoulder pain involves the rotator cuff, the group of muscles that steady the joint. When those muscles are irritated or weak, everyday movements start to hurt, but they usually respond well to the right loading program.

  • Pain reaching overhead or behind the back
  • Aching at night, especially lying on the shoulder
  • Weakness lifting or carrying
  • A painful arc partway through raising the arm
  • Stiffness getting dressed or grooming
Why it happens

  • Rotator cuff irritation or weakness

  • A fall or sudden strain
  • Poor shoulder blade control and posture

When to seek urgent care

Seek urgent care if the shoulder looks deformed after a fall, you cannot lift the arm at all, or the pain comes with chest tightness, sweating, or breathlessness.

Questions I hear a lot

Why does it hurt more at night?
Lying on the shoulder compresses the irritated tissues and there are fewer distractions, so the ache feels worse. It usually eases as the shoulder recovers.
Should I rest it in a sling?
Usually not. Keeping the shoulder gently moving within comfort prevents stiffness. I will guide how much is right for you.
How long until I can lift overhead again?
It depends on the cause and your starting strength, but a steady loading program is the fastest reliable route back.
