The basics

What causes hip pain?

Hip pain has many sources: irritation of the tendons and bursa on the outer hip, joint-related pain felt in the groin, or referred pain from the back. Where you feel it often hints at where it is coming from.

One very common type is pain on the outer hip from bursitis or gluteal tendon irritation, which flares with lying on that side or climbing stairs. Most hip pain responds well to targeted strengthening and load management.

What to notice

Common symptoms

  • Pain on the outer hip, groin, or buttock
  • Discomfort lying on that side at night
  • Pain on stairs, walking, or getting up
  • Stiffness after sitting
  • A weak or unstable feeling in the hip
Why it happens

Common causes

  • Gluteal tendon or bursa irritation
  • Weak hip and gluteal muscles
  • A sudden increase in walking or activity
  • Joint wear with age
  • Referred pain from the lower back

When to seek urgent care

Get urgent care if you cannot bear weight after a fall, the hip looks deformed, or pain comes with fever, or with unexplained weight loss or night pain that wakes you.

Questions I hear a lot

Why does my hip hurt when I lie on it?
That pattern often points to irritation of the tendons or bursa on the outer hip. It usually settles well with the right strengthening and positioning advice.
Is it coming from my hip or my back?
Sometimes back problems refer pain to the hip area. Part of my assessment is working out which, so I treat the right thing.
Will I need a scan?
Often not at first. I assess in person and refer for imaging only if it would change your treatment.
